Thank you for choosing Obstetrical & Gynecological Care. We look forward to serving you. To make your first visit as comfortable and positive as possible, please read the following information for our New Patients.
You may print and complete the new patient information forms in advance. For your first visit, please remember to bring the following with you to your appointment:

• Your insurance card
• Photo ID
• Any medications, prescription or over-the-counter that you are currently taking
• Co-pay, if you have one
• Completed Patient Questionnaire and Medical History forms

If you are seeing a provider for prenatal/pregnancy services,
please print and complete the following:

• Prenatal Questionnaire

If you do not have access to the internet or prefer to complete the patient information forms when you come for your appointment, please arrive approximately 15 minutes in advance.
